The Duodenal Switch is an excellent choice for patients seeking substantial weight loss. This procedure combines techniques from the gastric sleeve and RNY gastric bypass procedures, resulting in one of the most complex and effective weight loss procedures available.
Patients have been known to lose almost 80% of their excess weight within 12 months. The duodenal switch has also helped alleviate obesity-related health issues, such as type 2 diabetes and sleep apnea.
With over 15 years in bariatric surgery, Dr. Jacqueline Osuna is one of the few weight loss surgeons in Mexico capable of performing the duodenal switch procedure. Due to the extensive weight loss results of the procedure, Dr. Osuna typically reserves the Duodenal Switch for patients with a BMI of 45 or higher.
Duodenal Switch Surgery
The Biliopancreatic Diversion w/ Duodenal Switch (BPD/DS) procedure combines the restrictive and malabsorptive techniques from the gastric sleeve and the RNY gastric bypass surgeries. Similar to the gastric sleeve procedure, 70% of the stomach is removed to limit food intake and decrease the amount needed to feel full.
Additionally, the small intestines are rerouted and connected to the duodenum to allow food to bypass the upper portion of the small intestine. This alters how bile and digestive juices process food, reducing the absorption of calories, proteins, fats, and other nutrients, similar to the Roux-en-Y gastric bypass.
Single Anastomosis Duodenal Switch (SADi-S)
The Single Anastomosis Duodeno–ileal Bypass with Sleeve Gastrectomy, or SADI-S for short, is a version of the DS procedure that only uses one anastomosis to reroute and connect the small intestine to the stomach, rather than traditional 2. Dr. Osuna offers both versions of the DS procedure.
Complications of Duodenal Switch
Duodenal switch patients must follow a strict vitamin and nutrition plan after surgery. Because of the strong malabsorptive component of the DS, it can be difficult to absorb iron, proteins, and other nutrients, which can lead to anemia and gallstones.
Revision options are also not possible after the duodenal switch because of the complexity of the operation. For patients with severe obesity, skin removal may be necessary to achieve their ideal weight.
